10-Year Level Group Term Life Insurance - FAQs
Who is eligible for coverage?
You must be: 1.) A full-time, part-time, or retired federal employee under age 65; or, 2.) The spouse of a full-time, part-time, or retired federal employee under age 65, to apply for Starr Wright USA’s 10-Year Level Term Life Insurance.

Starr Wright USA enables you to determine how much life insurance coverage you need by using this Life Insurance Needs Calculator.
How can I apply for coverage?
You can download, complete, and return the signed application to the plan administrator at the address listed on the form. Be sure to answer all questions on the application.
If You Download the Application - Please Note: Your future Starr Wright USA premiums can be paid automatically from your account at your financial institution on a Monthly, Quarterly, or Annual basis through Electronic Funds Transfer (EFT). To sign up for EFT: download, complete, and return our EFT Form along with your completed application.

When will my coverage become effective?
Insurance will take effect on the first day of the month on or following the date your coverage is approved by ReliaStar Life, provided the initial premium is paid within 31 days.
When will my coverage end?
Coverage will end on the earliest of: the policy anniversary date on or after the insured person reaches age 75, or you fail to make payment of premiums when due; you request for such insurance to end; or if the group policy is terminated.
How often are premiums renewed?
Premiums remain level for an initial 10-year period.* At the end of the 10-year period, coverage will automatically renew without evidence of insurability. Renewal premiums are not guaranteed and will be based on your then attained age and will increase as you age. However, if you are under age 65 you may apply for a subsequent 10-year period at a guaranteed level rate based on your then attained age, subject to submission of satisfactory medical evidence.
*The initial premium will not change for the first 10 years unless the insurance company exercises its right to change premium rates for all insureds covered under the group policy with 60 days advance written notice.
